Chronic myeloid leukemia with a positive Philadelphia chromosome (CML Ph+) in the chronic phase. Chronic myeloid leukemia in the chronic phase, the acceleration phase, or the myeloid or lymphoid blast crisis phase with resistance or intolerance to previous therapy, including imatinib. – Acute lymphoblastic leukemia with a positive Philadelphia chromosome (Ph+ ALL) with resistance or intolerance to previous therapy.
